InterSystems, a creative data technology provider dedicated to helping customers solve the most critical scalability, interoperability, and speed problems, has today announced the launch of HealthShare Care Community\u2122. The solution means care teams can create a comprehensive shared care plan \u2013 improving communication, care transitions, and care coordination.<\/p>\n<p>With the power of InterSystems HealthShare behind it, HealthShare Care Community is helping patients, from the elderly to individuals living with long term, or chronic conditions, benefit from improved communication throughout their care journey. The solution creates comprehensive, flexible, and personalised care plans, combining input from the care team with additional wellness goals added by the patient. Plans can be customised to include wide ranging aims, from helping patients walk unaided for a family function, to end of life choices.<\/p>\n<p>When using HealthShare Care Community, everyone on the team can see who is contributing to the patient&#8217;s care, the role they serve, when they\u2019re involved, and their contact information. The entire care team \u2013 from patients and clinicians to GP\u2019s and community health workers \u2013 can easily contribute to a highly detailed, accurate and continually updated view of each patient\u2019s condition and care preferences, within their native workflow.<\/p>\n<p>&nbsp;<\/p>\n<p><a style=\"background-color: #00aa9e; padding: 20px; color: #fff; font-weight: bold; margin: 20px 0px 20px 0px;\" href=\"https:\/\/ec-projects.co.uk\/intersystems-uki\/wp-content\/uploads\/2020\/11\/InterSystems_Infograph_Healthshare5.1.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">Download Infographic<\/a><\/p>\n<p>&nbsp;<\/p>\n<p>\u201cHealthShare Care Community is built on the HealthShare Unified Care Record, meaning care plans can be shared across transitions of care and improve coordination across the care team,\u201d says Don Woodlock, Vice President Healthcare Solutions, InterSystems. \u201cNot only is patient data pulled together from disparate sources and care plans shared across the healthcare system, but the care providers can launch Care Community from within their own systems\u201d.<\/p>\n<p>With Care Community working in collaboration with the HealthShare product suite, providers can overcome the fundamental challenge for care teams that don\u2019t typically interact when spread across various care settings, with the ability to communicate through the HealthShare clinical message centre. Further, patients themselves can also log in via HealthShare Personal Community to communicate with care providers, get access to prescriptions and flag when they need help.<\/p>\n<p>Teams can also be more proactive with automatic notifications of patient-related events such as admissions, discharges, or transfers.
